Oddly enough the HD-A# that I bought had nothing inside the box about 5 free discs, so did a yahoo search and printed the PDF file that came up. I have it all set with a copy of the reset ready to go, just wanted to make sure that this was a correct form to mail out before I sent it.

The proper form is on the Toshiba website. If your form came from there you are good to go.

Bought my A3 in November and I'm still waiting on my discs. I've submitted my paperwork twice.

My discs just got here yesterday.
Now that the format is all but dead.

That's almost cruel.
