This, basically. Beyond that it's just a really unfortunate circumstance. It's entirely the fault of the parents (er.. grandparents(es?)) for not keeping in contact with their children and between siblings.

I'm no expert either, but our biology teacher told us that the rate for defects goes from around 2% to 25% when siblings reproduce. For first cousins it drops down to 12%.

A 1994 study found a mean excess mortality with inbreeding among first cousins of 4.4%.[53] A study of 29 offspring resulting from brother-sister or father-daughter incest found that 20 had congenital abnormalities, including four directly attributable to autosomal recessive alleles.[54]
