Balanced XLR Voltage levels - Hot and Cold pins

After trawling through XLR Balanced Voltages, one thing that seems to be 'incomplete' is the Voltage specification: For an un balanced Phono, stating say 2 Vrms, is perfect. But stating 2 Vrms for a balanced line is incomplete: About '0V', does this mean the Hot pin is 2 Vrms and the Cold pin is -2 Vrms (minus because it's out of phase) OR does this mean the Voltage across the Hot and Cold pin is 2 Vrms?

Another question is, what is the 'typical' rms Voltage - I have read that some go up to 6 V rms and beyond, and others state 1.095 Vpeak at 0dBu (again, is this for each pole or pole to pole?).
