Yamaha htr-6140 set-up problems

Greetings, I have a Yamaha HTR-6140 that I`ve been trying to fine tune for a while without decent results....stumbled upon these forums so I`m hoping somebody out there knows the tricks :).
Currently running the receiver with a Sanyo DP37647 37" LCD tv, a Samsung BD-P1500 blu-ray player, Xbox 360, some old RCA surround speakers from a former out-of-the-box surround set (which work just fine) and a Paradigm sub.
Let me explain how I have the connections set up and then my grievences, tho not overlly tech savvy so you`ll have to bear with me.
The speakers and sub are hooked up as per manual and all seem to work just fine, when they want to. I have HDMI cable from Xbox to the HDMI IN2 (dtv/cbl) port and this too seems to work ok when using "Game Playing" option (microsoft supplied cable). Hdmi port 1 (INT1 DVD) I have a Monster HDMI cable going to the blu-ray player. HDMI port 3 (OUT) has a Rocketfish HDMI cable to hdmi port 2 on back of tv.
Still with me? The receiver and tv are also hooked up with AV cables which go as follows: Red cable is from Audio dtv/cbl on receiver to Audio Output on tv. White cable is the same. Yellow cable goes from Monitor Out on receiver to Video (Video 1) on tv. Cable goes from wall to cable box, and from box directly to tv.
So, the problems.....first and foremost I cannot seem to get OSD at all, never have. When playing anything from blu-ray player it`s hit-and-miss. Some days it will function on surround with "DVD Playing" option and some days I need to activate 5-ch enhancer, but even that sometimes will not let itself be found. Xbox seems to work ok with surround at any time. Basic cable tv I can get through front speakers but not rear or sub.
I`ve tried playing around with different locations for cables but just can`t seem to find the proper setup to get everything functioning on a regular basis, would super appreciate ANY help on the matter. I realize I`m running some old equipment here, but it all sounds great when it`s working properly.
