Ideally, pull out and up. I haven't seen these for Tvs but have for PC (like this Amazon.com: 3 Way Adjustable Tilting Desk Mount Bracket for LCD (Max 33Lbs, 10~25inch) - Black: Electronics) but wold prefer on that mounts to the wall. If your TV is under the weight listed, you may be fine with any mount that matches up with your display. Ergotron has a number of solutions out there and is one of the few companies focussed on this type of VERY unusual setup. Quick Finder by Display Type: Results One in about 10,000 setups may ask for what you want.
