How are you connecting your DVR to the TV? If it's through HDMI you might need to run an audio out to the soundbar from the TV. You only should need to do this if your DVR cannot output audio through the optical out and the HDMI out at the same time.
Also make sure your DVR is outputting audio through the optical cable. An easy way to check this is to plug one end into the DVR and see if there is a little red light coming out the other side. If there is no light you need to configure your DVR to output audio through the optical output.
