In Wall/Ceiling Subs?

My buddy is building a house and i've been helping with the pre-wire and all that and helping him pick equipment. He just lost a major battle on the built in where his TV is going to be.

So now, we're going to have to go with bookshelf speakers... literally on a shelf of the build-in. Fine enough, but now we have an issue with the sub. There's no room on the side or corners to put it really, but may be some space on the tv wall, but to the left of the built-in.

1) How far outside of the Front Left & Right can it be? Normally i see the sub in between the front channels.

2) What about an in ceiling or in-wall sub?

3) What about under a coffee table a few feet in front of the built-in against the sidewall?

The guys are coming to wire tomorrow so he's a bit stressed. Thanks for the help guys.

In wall is a nice way to save space, but then you have to carefully pick the correct location as well. The spot where it is most convenient may not be the best for sound.
