Ear Health - Pain from IEM & Supra-aural but not circumaural?

I seem to have this long standing issue that I can't wrap my head around

IEM and Supra-aural headphones even at some of the lowest volumes, cause a sharp pain in my ears, especially after prolonged use. The same goes for phone receivers for land lines and cells.

Though circumaural even at higher volumes... no issues at all.

Would anyone happen to know why this might be?

If anyone would I trust this community would.

I would go to specialized doctor for diagnostic. From here you would at best get just a guess.
